Group Health Plan: The Hospital shall make available health insurance coverage as described below to all Residents, with an effective date of the first day of their Orientation This is a point of service plan with different levels of payment determined by the employee’s choices of providers. The employee contribution for this coverage will be determined annually.
All Residents have the option of participating in an IRS Section 125 plan that allows employees to pay their health insurance premium contribution on a before-tax basis.
Dental and Vision Insurance: Subject to the provisions of the master agreements, the Consortium shall make available to all Residents dental and vision insurance coverage under its current contracts at Resident’s expense. The Hospital reserves the right to change insurance carriers, self-insure or self-administer any dental or vision benefit arrangement. The employee contribution for this coverage will be determined annually.
Group Life Insurance Plan: Subject to the provisions of the master agreement, the Hospital shall make available and pay full cost of life and accidental death and dismemberment (AD&D) insurance to all Residents. Term insurance is provided in the amount of $300,000 for the Resident. Supplemental life insurance may be purchased by the Resident. If Resident elects Supplemental Life Insurance for self, he/she may also purchase life insurance for spouse and dependent children. The Hospital reserves the right to change insurance carriers, self-insure or self-administer any life and AD&D benefit arrangement provided the value of the benefits under any new arrangement are comparable with the arrangement in effect prior to the change. Long-Term Disability: Subject to the provisions of the master agreement, the Hospital shall make available and pay full cost of long term disability insurance coverage under its current contract, and pay the full cost. Present disability insurance takes effect 91 calendar days after the Resident becomes disabled by accident or sickness, and provides a monthly income of 60% of the Resident’s salary (up to a maximum monthly benefit of $7,000). This benefit is integrated with any social security disability benefits. The Hospital reserves the right to change insurance carriers, self-insure or self-administer any disability benefit arrangement provided the value of the benefits under any new arrangement are comparable with the arrangement in effect prior to the change.
